A field certification clerk from the Cherokee Nation Registration Department will be available to assist Cherokees with applications for Certificate of Degree of Blood (CDIB) cards and membership in the Cherokee Nation at sites throughout the tribes 14-county area.The registration department is located in the W.W. Keeler Tribal Complex in Tahlequah, and is open from 8 a.m. to 5 p.m., Monday through Friday. There is no charge for application services or tribal membership. The sites and schedule follows.

Persons who have had problems obtaining necessary documents for these applications are asked to bring all appropriate paperwork to the field registration sites. Applicants should make sure their applications are complete and accompanied by the proper documents by contacting the office for instructions and current forms.
Old forms are no longer acceptable. Due to the Privacy Act, authorization to release information and affirming true facts are part of the new forms. Certain records may be needed to obtain a CDIB for a child even though a parent may already have the card.
To obtain a CDIB, applicants must know the roll number of the original enrollee to whom they are related. An original copy of a state-certified birth certificate or a delayed birth certificate linking the applicant to the enrolled ancestor must be attached.
If a delayed certificate is used, an additional document such as an affidavit of personal knowledge and memory, must accompany the application. Applicants should submit state-certified birth or death certificates for all non-enrolled ancestors, and complete an application for membership in the Cherokee Nation.
